Welcome to the APS Admission Team Page

Everyone in the admissions department is dedicated to making the referral process as smooth as possible for your office. On this page you will find everything you will need to refer a patient to our office. When a referral is received we understand it is a referral but we treat it as the doctor’s order. We will contact the referred patient and schedule them at the earliest date possible. A team member will notify your office of the date and time of the appointment. We always strive to schedule the patient prior to the patients medication refill date so the referring physician will not have to consider writing a bridge prescription for the patient. We also have a full time staff person dedicated to sending treatment notes back to your office as well.

  • Patient DEMO( including name, DOB, SS, insurance card and #, contact #)
  • Diagnosis of pain ( Chief Complaint)
  • Copies of insurance card
  • All notes referencing pain medication such as notes from other pain mgt clinics the patient has been to in the last 12 months
  • Last blood work or Urinalysis
  • Radiology reports such as x-ray or MRI findings is most important
  • Once the referral team receives this information we will call you patient immediately to get them on the schedule as soon as possible. If some part of the information is missing we will reach out to the primary care office to collect the missing information.

Insurances Accepted

At Associated Pain Specialists we accept most major insurance providers. If you do not see your provider listed please call the office for more information.
