Associated Pain Specialists is the trusted pain mgt referral source for over 300 primary care offices in East Tennessee. Our team of admission specialist are here to make the referral process an efficient and easy process.

The majority of referrals to APS are in reference to opioid management. For this reason our team of providers request certain patient history to assess if taking over the prescription is justified.

The following information MUST be attached to all referrals for our admission team to go to work. Once we receive your referral we will contact your patient and schedule them as quickly as possible. Please remember in order for us to contact your patient immediately our physicians require the following information be in the patient chart prior to your patient being scheduled.

  • Patient DEMO( including name, DOB, SS, insurance card and #, contact #)
  • Diagnosis of pain ( Chief Complaint)
  • Copies of insurance card
  • All notes referencing pain medication such as notes from other pain mgt clinics the patient has been to in the last 12 months
  • Last blood work or Urinalysis
  • Radiology reports such as x-ray or MRI findings is most important
  • Once the referral team receives this information we will call you patient immediately to get them on the schedule as soon as possible. If some part of the information is missing we will reach out to the primary care office to collect the missing information.
